* PCA USA, LLC or employees/Contractors will not be responsible for lost, stolen, or damaged personal items.
* The airbrush tanning process has been explained to me and I hereby agree to allow PCA USA ,LLC employees/contractors to apply spray tan solution to my skin using the airbrush tanning process.
* To my knowledge, I have no medical condition or allergy which would preclude me from having this procedure done.
* I agree to hold harmless PCA USA, LLC and it‘s employees/Contractors of any medical complications that may arise and I have been advised to discontinue use if any reaction occurs.
* I understand that the results of airbrush tanning do vary and that no guarantees of specific results are offered or implied.
* I hereby release this establishment, as well as the airbrush technician, and hold them harmless from any liability involved in the use of or arising during the tanning process. In consideration of receiving permission to participate in any activities sponsored by or provided on the premises of PCA USA, LLC, or to utilize or otherwise obtain any services, products or goods offered or sold by, or on the premises of the PCA USA, LLC, the receipt of such permission being also acknowledged, each of the undersigned releases PCA USA, LLC, its members, managers, agents, officers, servants, and employees of and from any and all liability, claims, demands, actions and causes of action whatsoever, arising out of or related to any loss, damage, or injury to person or to property (including clothing), including death, that may be sustained by any or each of the undersigned, or any property of any or each of the undersigned, while in or on the premises, or any premises leased to, owned by, sanctioned by, or under the control or supervision of PCA USA , LLC or en route to or from any of the premises.
Each of the undersigned being duly aware of the risks and hazards inherent in entering on the premises and/or in participating in any activities involving PCA USA, LLC, voluntarily and knowingly assume all risks of loss, damage, or injury, including death, that may be sustained by any or each of the undersigned arising from the application of the spray tan products.
This release is binding on the undersigned heirs, next of kin, executors, administrators, and personal representatives of each of the undersigned. If under 18 a parent or legal guardian MUST agree consent for this form.
I have read, understood, and freely signed this agreement. I further certify that I am at least eighteen years of age, or, if under eighteen years of age, my parent or legal guardian has signed on my behalf,and that I am otherwise legally competent to sign this agreement. I further understand that the terms of this agreement are legally binding. This agreement shall be construed and enforced in accordance with the law and I consent to the jurisdiction of said state. If any portion of this agreement is held invalid, it is agreed that the balance shall continue in full legal force and effect.
